hggtk/thgconfig: add write back and history
History is stored in ~/.hgext/tortoisehg in a shelve db.
Apply button still needs to be greyed when there is nothing
to save.

hggtk/thgconfig: add --focusfield argument
Apps can request the dialog to switch to an arbitrary
tab/page and select (focus) the specified configurable.
These are useful hints to the user. For instance, the
commit tool could launch thgconfig with focusfield='ui.username'

Paths tab and hgmerge tab will need special logic to
maintain a list of configurables. Still need to implement
write back and persistent history.
